SESP Ficha Resumo_CCR Form Specification and Requirements_1.3

This document outlines all the fields that need to be created in the SESP CCR Summary Form, according to the existing data types and instructions for completion of the paper-based form – page 1. The Ficha Resumo CCR will only be available for data entry for patients with a CCR NID registered in the system.

 

Este documento descreve todos os campos que precisam de ser criados na Ficha Resumo CCR no SESP, de acordo com os tipos de dados existentes e instruções para o preenchimento do formulário em papel – página 1. A Ficha Resumo CCR estará disponível para digitação apenas para os utentes com NID de CCR registado no sistema.
