The purpose of this document is to describe the requirements and specifications of the Ficha da Doença Avançada por HIV (DAH). The DAH Form (Ficha DAH) captures data related to HIV advanced disease, which is defined by WHO as a patient with a CD4 cell count <200cells/mm3 or WHO stage 3 or 4 in adults and adolescents. All children younger than five years of age are considered to have advanced HIV disease. The DAH form has the following two objectives: 1. Capture data for relevant analyses of indicators related to advanced disease (monitoring and evaluation) as well as 2. Longitudinal follow-up of HIV+ patients with advanced disease, to ensure higher quality follow-up and continuity of care (quality of care for individual patients). The form is a new instrument, which was piloted by the PNC ITS-HIV/SIDA in 2022 and will be gradually expanded in 2023.
